Pass as_of with the encounter's date of service to check the code was in effect on that date — ICD-10 sets change every October 1. Codes retired from the current set still resolve, marked with the year they were retired.
Search ICD-10-CM diagnosis and ICD-10-PCS procedure codes. Unified tool that supports searching by code or description. Use this for all ICD-10 search needs. Serves the FY2026 code set (in effect Oct 1 through Sep 30). Limits
- It answers about codes, never about people. No tool takes a clinical note, patient, encounter, provider, payer or claim parameter. The union of all inputs across all six tools is a code or query, a code-type and search-field selector, a HIPAA-validity filter, a limit, a prefix, a chapter or category, a body-system and section character, and an optional date of service. Do not paste patient information into a prompt — the connector has nowhere to put it.

Setup instructions and tool names are not interchangeable between them. - US code sets only, one fiscal year at a time. ICD-10-CM and ICD-10-PCS are the United States clinical modification and procedure system. WHO's international ICD-10 is a different classification with different codes, and ICD-11 is a different revision entirely. Neither is served here, and neither are CPT or HCPCS.
- FY2026, with FY2027 already published. The server reports the set in effect 1 October 2025 to 30 September 2026. CMS has published the FY2027 files effective 1 October 2026. Re-check what the server reports after that date rather than assuming continuity.
- The October-only update claim is incomplete. Tool descriptions state the sets change every October 1. CMS also publishes April 1 update files, present for FY2024, FY2025 and FY2026. A mid-year revision can change what is billable inside a fiscal year.
as_ofhistory is bounded. Diagnosis history runs from FY2020 and procedure history from FY2021. An encounter before those years cannot be date-checked through this connector.- HIPAA validity is not coverage.
valid_for_hipaa_transactionsdistinguishes a billable code from a category header. It says nothing about whether a payer will reimburse, which depends on coverage policy, medical necessity and documentation that this server cannot see. - Results are capped and broad queries truncate.
search_codesreturns 20 by default and 200 at most;get_hierarchyreturns 500 by default and 2,000 at most, and its description warns that broad prefixes may be truncated. Treat a chapter-level result set as a sample. - No combination checks, no guidelines, no crosswalks. The connector returns individual codes. It does not validate code pairs, return the ICD-10-CM Official Guidelines or the tabular list's instructional notes, or map between ICD-10 and ICD-9, ICD-11, CPT or HCPCS.
openWorldHintis absent on every tool. The server makes no statement about corpus openness in either direction. Anthropic's tutorial resolves the question — a stored database of published code sets — but that is documentation, not an annotation, and it is attributed as such.- The server's own instructions name tools that do not exist. - Anthropic calls the healthcare bundle experimental. Its own README states the plugin is provided as-is for evaluation against test or sandbox systems, is not validated for clinical use, is not a medical device, and should not drive patient-care or coverage decisions without qualified human review.
- No tool writes anything. All six are annotated read-only, non-destructive and idempotent. The connector cannot submit a claim, amend a record or change the classification.
- We did not call any tool. Our check was a read-only
initializeandtools/listhandshake. Everything above about tool behaviour comes from the server's own descriptions, schemas and instructions, plus Anthropic's published tutorial, plugin manifests and skill, plus CMS's and CDC's own pages. No code was looked up and no patient information exists anywhere in this work.

Is this the same as the Agentman ICD-10 Medical Coding connector?
No. Two different servers share the ICD-10 name. This page describes Anthropic's connector at hcls.mcp.claude.com/icd10_codes/mcp, listed at anthropic tier with the six tools below. Agentman operates a separate ICD-10 Medical Coding server at mcp.agentman.ai/icd10/mcp with its own eleven tools under icd10_ prefixed names. Different operators, different endpoints, different tool sets.
Can the ICD-10 connector assign the right code for a patient encounter?
No. It looks codes up in a published classification and reports whether each one is valid for HIPAA transactions. No tool takes a clinical note, a patient, an encounter or a payer. Code assignment is a coding decision made by a qualified coder applying the ICD-10-CM Official Guidelines to documentation, and this connector supplies reference material for that work rather than performing it.
Which ICD-10 code sets does this connector actually serve?
Two United States code sets: ICD-10-CM for diagnoses and ICD-10-PCS for inpatient procedures. The server reports the FY2026 set, in effect from 1 October 2025 through 30 September 2026. It does not serve the World Health Organization's international ICD-10, which is a different classification, and it does not serve ICD-11, CPT, HCPCS or any other code system.
Does the ICD-10 Codes connector need a sign-in or an API key?
No. We opened an anonymous handshake against the endpoint on 2026-08-18 and it returned all six tool definitions with no credential of any kind. Anthropic's directory records the server as authless with a no_auth posture, and the host publishes no OAuth protected-resource descriptor at either the root or the path-suffixed location we probed.
How do I check a code was valid on the date of service?
Pass the encounter date to the as_of parameter, which lookup_code, search_codes and validate_code all accept as a YYYY-MM-DD string. The response then reports whether the code was in effect in the fiscal year covering that date. Coverage runs back to FY2020 for diagnosis codes and FY2021 for procedure codes, so older encounters cannot be checked this way.
Does a code being valid for HIPAA transactions mean the claim will be paid?
No, and the two questions are unrelated. The valid_for_hipaa_transactions flag says only that a code is a fully specified billable code rather than a category header. Whether a payer reimburses a claim depends on coverage policy, medical necessity, documentation and the plan, none of which this connector can see. It answers a classification question, not a payment question.
Do the ICD-10 code sets really only change on 1 October?
No. The connector's own tool descriptions say ICD-10 sets change every October 1, and that is the main annual update, but CMS also publishes mid-year files effective April 1. Its ICD-10 page carries April 1 updates for FY2024, FY2025 and FY2026. Treat the October date as the major revision and check CMS for interim changes before relying on a code set boundary.
